HAVERHILL– Carol Sue Polachek, 49, of Haverhill, formerly of Greenup County, Ky., died Sunday, Feb. 20, 2011, at her home. She is survived by her husband Richard Polachek. There will be no services. Arrangements are by Reed Funeral Home in Greenup, Ky.
