FLATWOODS, Ky. — Phyllis Ann Meadows, 51, of Flatwoods, died Tuesday, July 12, 2011, at home. She was the wife of Gerald Lee Meadows, who survives. Funeral will be at 11 a.m. Friday at Carman Funeral Home with Pastor Lloyd Bailey. Burial will be in Bellefonte Memorial Gardens. Visitation is 6-9 p.m. today at the funeral home.
